At the International Symposium, held in Monaco in 1954, the term "non-gonococcal urethritis" was adopted, denoting an extensive group of inflammations of the urethra, different in etiology and pathogenesis, except those caused by gonococci. In most cases, non-gonococcal urethritis (NGU) is classified as a sexually transmitted infection (STI), the number of which increases markedly from year to year, taking, according to the report of the WHO Scientific Group, "epidemic" . In 1995, 600-700 million patients suffering from chlamydial urogenital infection were registered in the world , and the economic losses from chlamydial diseases in the US are estimated at 1 billion dollars a year. Early onset of sexual activity, promiscuity, lack of awareness of the rules of contraception, sexual excesses are the reasons for the rapid growth of STIs observed in Russia over the past decade. In 1998, according to the Ministry of Health of the Russian Federation, the number of patients with urogenital chlamydia and trichomoniasis was already 113.8 and 317.2 per 100 thousand of the population, respectively.

According to the severity of clinical signs, 3 main forms of urethritis are distinguished: acute, torpid and chronic (Porudominsky I.M., 1963).


The mechanism of development of the inflammatory process in the urethra is determined by the state of its mucosa. The antimicrobial resistance of the urethral mucosa depends on its mechanical integrity, the production of a sufficient amount of mucus by secretory epithelial cells and a certain biochemical composition of urethral mucus that has an antimicrobial effect (immunoglobulins, lysozyme, etc.). In case of violation of the integrity of the mucosa, microorganisms are introduced into the zone of its defeat, an inflammatory process develops.

Clinical manifestations of acute urethritis are abundant discharge from the urethra; pain, itching and burning in the urethra, redness and swelling of the lips of its outer opening, pain during urination. The clinical picture of torpid and chronic urethritis is less pronounced and is characterized by discomfort and itching in the urethra, paresthesia in the glans penis. Discharge from the urethra disturbs only in the morning or is absent altogether. However, in the first 2 months the presence of such symptoms is called torpid urethritis, and with a further course - chronic.

Traditional methods of identification Chlamydia trachomatis are its isolation in cell culture, detection of chlamydia antigen by direct immunofluorescence (DIF) with monoclonal antibodies, enzyme immunoassay and polymerase chain reaction (PCR). In this case, the verification of the diagnosis is based on the detection of chlamydia using two methods, one of which is PCR.
laboratory studies necessary for the diagnosis of trichomoniasis include: the study of the native drug, microscopy of the drug, stained with 1% solution of methylene blue or brilliant green and Gram, as well as cultural examination. Moreover, verification of the diagnosis is possible upon detection Trichomonas vaginalis one of these methods. It should be noted the need for simultaneous treatment of sexual partners and the use of barrier methods of contraception during sexual intercourse during therapy in the identification of these pathogens.

Currently, antimicrobial therapy that is effective against chlamydia is carried out with drugs from the groups of macrolides, tetracyclines and fluoroquinolones. Moxifloxacin is one of the antimicrobial drugs from the group of fluoroquinolones successfully used recently in urogenital chlamydia. Nitroimidazoles continue to be the only group of antimicrobial drugs for the etiotropic therapy of trichomoniasis: metronidazole, tinidazole, ornidazole have a selective bactericidal effect against microorganisms whose enzyme systems restore the nitro group. Active reduced forms of drugs disrupt DNA replication and protein synthesis in a microbial cell, and inhibit tissue respiration. Trichomonas remain sensitive to nitroimidazoles, including metronidazole, which remains the drug of choice for the treatment of urogenital trichomoniasis.


Material and methods

In the clinic of urology of the Russian State Medical University, 36 men with urethritis caused by Chlamydia trachomatis and Trichomonas vaginalis. The age of the patients ranged from 19 to 46 years. In 15 (41.7%) patients, only chlamydial infection was found, in 9 (25%) - only trichomonas infection, in 12 (33.3%) - a combination of chlamydial and trichomonas infections was detected. All patients underwent physical examination, digital rectal examination, transabdominal and transrectal ultrasound examination of the prostate gland and laboratory tests (general blood and urine tests, bacteriological examination of urine, microscopic analysis of prostate secretions). The infection was determined in scrapings and smears from the urethra, the identification of pathogens was carried out by polymerase chain reaction (PCR) and direct immunofluorescence (DIF); trichomonads were detected in the study of the native preparation and microscopy of the preparation stained with 1% methylene blue solution.

When clarifying the complaints, it turned out that 21 patients were concerned about the presence of discharge from the urethra, 25 patients had burning, itching and paresthesia of the glans penis, 30 patients had redness and swelling of the lips of the external opening of the urethra, 14 patients had frequent painful urination. As a result of a comprehensive examination, one could assume urethritis in 22 patients, a combination of urethritis and prostatitis in 14 patients.
in all patients, urine culture for the “banal” microflora turned out to be sterile, and in 18 patients, more than 10 leukocytes were found in the secretion of the prostate gland in the field of view. In patients with a picture of the inflammatory process in the prostate gland, ultrasound examination showed echo heterogeneity of the gland with the presence of focal or diffuse hyperechogenicity along with an increase in the volume of the prostate gland (the average volume of the prostate gland was 34.5 cm3).

15 patients with chlamydial urethritis received moxifloxacin 400 mg (1 tablet) once a day, regardless of food intake. 9 patients with trichomonas urethritis received metronidazole 500 mg (2 tablets) every 12 hours with meals. 12 patients with urethritis of mixed etiology took both of the above drugs in similar dosages. The duration of treatment was 10 days.

Along with antimicrobial drugs, patients were prescribed anti-inflammatory therapy (rectal suppositories with diclofenac 100 mg at night), as well as enzyme preparations and vitamins for 10 days. Patients with prostatitis underwent low-intensity transrectal laser therapy. Rectal exposure to laser radiation (ALT "Mustang-2000" apparatus) was carried out in a biomodulated mode, in the red optical range with a wavelength of 0.63 μm, a power of 20 mW. The exposure time was 10 min. The course of laser therapy consisted of ten procedures. All patients were warned about the need to use contraceptives during sexual relations in order to avoid superinfection. Monitoring the effectiveness of therapy using PCR and PIF methods for chlamydial and mixed urethritis, studies of native preparations and microscopic examination of stained preparations (for Trichomonas and mixed urethritis) was carried out 2 weeks after the end of the course of treatment.


results

At the control examination of all 36 patients, carried out two weeks after the end of the course of therapy, Chlamydia trachomatis ceased to be detected in scrapings from the urethra by PIF and PCR in 14 (93.3%) of 15 patients treated for only chlamydial urethritis; Trichomonas vaginalis was not found in the study of native and stained preparations in all 9 (100%) patients with only Trichomonas urethritis, and in the group of patients with mixed infection, both microorganisms were not found after the course of treatment in 10 (83.3%) of 12 men. It should be noted that a patient with a newly discovered PIF 2 weeks after treatment with chlamydial infection was prescribed a ten-day course of an antibiotic from the macrolide group (josamycin 500 mg 2 times a day before meals). At the control examination after 2 months. the pathogen in the scraping from the urethra was not detected by PIF and PCR. In 2 patients with mixed urethritis after a course of treatment with moxifloxacin and metronidazole, a follow-up examination revealed only Chlamydia trachomatis, that is, the infection mixed before the start of therapy passed after treatment into a monoinfectious form. These patients were also prescribed a ten-day course of josamycin, which was sufficient to eliminate the pathogen. At the control examination after 2 months. in all 36 men, chlamydia and trichomonas were not detected.


Taking moxifloxacin and metronidazole led to a significant reduction in symptoms, the disappearance of inflammatory changes in the secretion of the prostate gland in patients with manifestations of prostatitis, and a decrease in the volume of the prostate gland, which was shown by ultrasound data. The dynamics of the above parameters before and after treatment is shown in Tables 1 and 2.

Thus, the therapy of patients with chlamydial and trichomonas urethritis with moxifloxacin and metronidazole led to a significant reduction in symptoms, a decrease from 34.5 to 22.6 cm3 in the average prostate volume and normalization of the number of leukocytes in the secretion of the prostate gland in most patients.

Discussion

The study has demonstrated high sensitivity of chlamydial infection to metronidazole and moxifloxacin , which showed high efficiency in monotherapy of chlamydial urethritis and in complex antimicrobial therapy of chlamydial-trichomonas urethritis in men with a minimum level of side effects throughout the course of treatment. The microbiological efficacy of moxifloxacin in 27 patients with chlamydial and mixed urethritis was 88.8%, the microbiological efficacy of metronidazole therapy in 21 patients with trichomonas and mixed urethritis was 100%. During a short course of antimicrobial therapy, it was possible to achieve effective elimination of the pathogen and a significant reduction in the symptoms of the disease.


Conclusion

Based on the study of the efficacy and safety of moxifloxacin and metronidazole preparations, their high activity against pathogens of urogenital infection (chlamydia and Trichomonas) with a low level of side effects was established. When prescribing drugs for 10 days, it was possible to achieve eradication of the pathogen or the transition of a mixed form of infection into monoinfection. The microbiological efficacy of moxifloxacin and metronidazole was 88.8% and 100%, respectively.

Urogenital chlamydia is one of the serious medical and social problems. This infection has become widespread in various countries of the world, and the number of infected is constantly growing.

Recently, more and more data have appeared on the possibility of the existence of latent or persistent forms of chlamydia both in in vitro modeling and in the patient's body in vivo.

Clinic of urogenital chlamydia

Currently, domestic experts have not come to a consensus on the classification of chlamydia. Some of them use a classification similar to that adopted for gonorrhea, others are arbitrary, in accordance with the localization of lesions or the nature of the infection. It is advisable to use the term "urogenital chlamydia" when making a diagnosis, which makes it possible to identify chlamydia among sexually transmitted diseases.

The clinical picture of diseases of the genitourinary organs caused by chlamydia differs from gonococcal infection mainly by the lower severity of the inflammatory process, the high frequency of complications and resistance to therapy. For women, chlamydia is the most dangerous, because it causes not only a serious pathology of the lower genitourinary system, but is often accompanied by the development of an ascending disseminated infection, the occurrence of pelvic inflammatory diseases, the development of infertility, infection of newborns. Most often and first of all, the cervix is ​​​​infected with chlamydia, from where, as from a reservoir, the process spreads to the urethra, uterus and fallopian tubes, to the internal organs. The incubation period for chlamydia varies between 15-30 days.

A feature of the clinical course of the inflammatory process in chlamydia of the genitourinary organs is an asymptomatic and oligosymptomatic course, multifocality: urethritis, epididymitis, oophoritis, prostatitis, vulvovaginitis, proctitis, colpitis, cervicitis, endocervicitis, salpingitis, parametritis, septic conditions. The acute form of chlamydia with profuse discharge, severe hyperemia of the mucous membranes, with dysuric disorders and severe pain is rare even in men, and in women only in exceptional cases. Subacute and torpid forms are much more common. In men with chlamydia, examination reveals moderate hyperemia and swelling of the sponges of the urethra, sometimes the foreskin of the penis, mucopurulent discharge. In women, chlamydia is manifested by increased pain, pain in the lower abdomen and in the sacrum during and after intercourse, pain during urination. The disease often proceeds without subjective sensations and with minimal clinical manifestations or without them.

The symptomatology of chlamydial diseases in children is more pronounced and, depending on the duration of the inflammatory process, manifests itself in the form of vulvovaginitis, urethritis, cervicitis. Girls get sick more often, especially at an older age, when infection occurs sexually. Along with diffuse hyperemia of the vulva and adjacent areas of the skin, there is swelling of the external genital organs, their maceration as a result of abundant, liquid gray-green discharge. When the urethra is affected, the sponges are hyperemic, edematous, covered with gray discharge, possibly frequent and painful urination.

In pregnant women with chlamydia, complications are more often observed: urgent delivery, premature birth, non-developing pregnancy, spontaneous miscarriage. It is generally accepted that a newborn in 40-50% of cases acquires a chlamydial infection during childbirth due to infection with pathological material when passing through the birth canal of a mother with chlamydia (chlamydia was isolated from newborns from the conjunctiva, nasopharynx, middle ear, trachea, lungs, rectum and vagina), but it is also possible for the infection to enter the amniotic fluid and intrauterine infection of the fetus.

Methods for laboratory diagnosis of urogenital chlamydia

The basic principles for diagnosing chlamydial infection are the same as for other bacterial pathologies. Test procedures include the following:

  • Direct visualization of the agent in clinical samples when stained (bacterioscopic method). Bacterioscopic studies involve the identification of the morphological structures of chlamydia in the affected cells (clinical material).
  • Determination of specific chlamydial antigens in clinical specimens. Bacterioscopic studies involve the detection of chlamydia antigens in the affected cells (clinical material). In routine laboratory practice, both direct (PIF) and indirect immunofluorescence (NIF) methods can be used. The first method involves the treatment of the drug directly with specific mono- or polyclonal antibodies labeled with fluorescein. When using the second method, the drug is treated first with serum containing unlabeled anti-Chlamydial antibodies, and then with anti-species fluorescent serum.
  • Direct isolation from the tissues of the patient (bacteriological method). Bacteriological studies involve the detection of chlamydia in the affected cells (clinical material).
  • Serological tests that detect antibodies.

Methods for the serological diagnosis of chlamydia are based on the determination of specific antibodies in the blood serum, as well as in the secrets of persons with or with a history of chlamydia. Interpretation of the results of a serological examination should be carried out in conjunction with an analysis of clinical and epidemiological data, as well as taking into account the characteristics of the serological test used.

For serodiagnosis, enzyme-linked immunosorbent assay (ELISA for the presence of antibodies) is currently most often used. The general principle of ELISA diagnostics is as follows: the antigen is fixed on a solid surface, treated with the test serum, and then with anti-species immunoglobulin associated with the enzyme, which is visualized after the addition of the substrate. The advantage of the method is the ability to automatically record the results and identify classes of antibodies - IgG, IgA, IgM. Given the low immunogenicity of "urogenital strains" of chlamydia and the possibility of the presence of antibodies after a previous chlamydial infection, it is necessary to examine several serum samples in the course of the disease with an interval of 2-3 weeks.

The commonly used method of serodiagnosis is the reaction of indirect immunofluorescence for the detection of antibodies (NIF). When conducting NIF, fixed purified chlamydia antigens are used, applied in the form of dots on glass. The applied serum of patients reacts with antigens of various serotypes, after which it is treated with anti-species luminescent serum. The test is sensitive, in many cases it provides information about the chlamydia serotype; it is advisable to use it in epidemiological studies.

  • Determination of specific chlamydial genes in clinical specimens.

Nucleic acid amplification methods

DNA diagnostic methods are based on the complementary interaction of nucleic acids, which makes it possible to identify the nucleotide sequence in the genes of the desired microorganism with high accuracy. Of the numerous modifications of this method, the polymerase chain reaction (PCR) and the ligase chain reaction should be singled out as the most widely used.

Treatment of urogenital chlamydia

Therapy of diseases of the genitourinary tract caused by chlamydia is based on the use of drugs that are active against this microorganism. At the same time, the biological characteristics of chlamydia should also be taken into account.
